What to include

A result looks wrong: the store address, what CheckoutMap showed and what you expected.
Subscription or billing: the email you used to pay, or your access code (CM-…).
Lost access code: email us from the address you paid with and we'll look it up for you.
An app we don't recognize yet: its name and a store that uses it.
